Your Personal Information
When you use Pro-Change’s Youth LifeStyle programs, if you are younger than 13 years of age, the only identifying information we ask for is your date of birth. If you are 13 years of age or older, we ask for first and last name in addition to date of birth. We ask for your full name so that we can create reports of who has participated for your organization. Pro-Change does not share, disclose, or sell e-mail addresses or other Personal Information that we collect. We use the identifying information solely for the purposes described below (see “Disclosures to You”). Pro-Change will not sell any Personal Information under any circumstances.
Disclosures to You
If you are 13 years of age or older, we may use your first and last name to create reports on who has participated within your organization. This program is exempt from the Children’s Online Privacy Protection Act (COPPA) because it does not collect personal information of any sort from children under the age of 13. The primary goal of COPPA is to place parents in control over what information is collected from their young children online.
Required by Law. We will disclose Personal Information if required to by law, such as in response to a court order.
Disclosures to our Customer
You are participating in Pro-Change’s Youth LifeStyle programs through one of our customers (your health plan, employer, school). We will provide our customer aggregated information (group information without individual names attached).
This website is intended for use by children and adolescents. However, Pro-Change does not knowingly collect personal information from children under 13 years of age. If you are under 13 years of age and have disclosed Personal Information to us through this site, please contact us (using the information listed below) so that we can delete your Personal Information.
The Pro-Change Youth LifeStyle programs use industry-standard cookies to keep track of user session data in your web browser. This allows you to resume a session if a browser window is closed by accident and it allows our website to recognize you when you come back to the site for another session. Cookies allow Pro-Change to better customize the user’s experience with the program. To use our Youth LifeStyle programs, you will need to enable cookies, if they are turned off on your computer.
We use a number of administrative security procedures (such as workforce clearance and information management policies), physical security measures (such as locked doors and screens for monitors), and technical security safeguards (such as automatic logoff and passwords) to protect your Personal Information. Of course, due to the nature of the Internet and online communications, we cannot guarantee that all information we collect will remain absolutely safe. We are not liable for the illegal acts of third parties, such as criminal hackers.
